Possibly from boter (“butter”) + *ramme, *remme (“thick slice of bread”) (compare obsolete rammel, remmel (“thick slice of bread”) and West Flemish rammeke (“toasted sandwich”)). Alternatively perhaps from boter (“butter”) +‎ *ham (“chunk”). Alternatively, short for boterenbroot (“buttered bread”). Unrelated to ham (“ham”).
