Most likely a derivation from gnjȉo / гњи̏о (“rotten”), compare archaic gnjilan / гњилан (“muddy”), because of the muddy soil characterising of the town. Traditionally the local urban pronunciation was Gilane, presumably by influence of the Albanian and Turkish forms which dropped the /ɲ/, see Albanian Gjilan, while in the neighbouring countryside it was retained as Gnjilane, which is the form that was ultimately chosen as standard.
